Medical Forms

At St Francis of Assisi, the staff are always more than happy to work with parents and carers to provide support in the care of the student's medical welfare during school hours. In order for us to do so effectively, we ask that you keep us fully informed and complete the appropriate paperwork. All medication must be delivered to the Front Office with the accompanying dispensation of medication form so that it can be safely stored and dispensed at the appropriate time. 

Request to Dispense Medication During School Hours - If your child requires medication to be administered at school please complete the following form and deliver it to the Front Office with the medication.

Medical Advice to School from the Student's Doctor - Medical forms are to be completed by a Student's Doctor when there are ongoing healthcare issues that require regular administering of medication during school hours.

Action Plans are required for students with diabetes, anaphylaxis, epilepsy, asthma, and any other significant medical conditions requiring careful monitoring and support during school hours. These plans are to be completed by the student's doctor using one of the following forms or the doctor's own form if preferred.
